The inquiry form helps Bri understand what kind of support you may be looking for, what your family is carrying right now, and what next step may make sense.

This form is for

  • Sharing what daily life feels like right now.
  • Describing routines, transitions, care needs, or caregiver capacity you want support with.
  • Starting a low-pressure conversation about what kind of support may make sense.

This form is not for

  • Emergency requests or crisis response.
  • A full intake form, service agreement, medical form, or guarantee of services.
  • Detailed medical, legal, custody, or crisis information.
